Position Overview We are seeking a skilled and motivated Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The ideal candidate is reliable, organized, confident in clinical decision-making, and passionate about helping patients achieve their health goals. Responsibilities

Perform in-home patient assessments and skilled nursing visits Develop and update individualized care plans Perform wound care, disease management, medication administration, and patient education Communicate effectively with physicians, case managers, and interdisciplinary teams Complete accurate, timely documentation in accordance with regulatory requirements Participate in on-call rotation as needed Build trusting relationships with patients and families Qualifications Current RN license in the state of Texas 1+ year of nursing experience (Home Health preferred but not required) Strong clinical assessment and critical-thinking skills Reliable transportation, valid driver’s license, and proof of auto insurance Comfortable using EMR/mobile charting Compassionate, professional, and patient-focused What We Offer Competitive pay + mileage reimbursement Flexible scheduling and autonomy in your daily workflow Supportive leadership and responsive office staff Opportunities for growth and advancement Paid training and ongoing clinical support Powered by ExactHire: 189341 Show more Show less
