About this role
Registered Nurse Home Health position at Elara Caring providing high-quality care to patients in their home setting. Responsibilities include assessing assigned case load of home health care patients, maintaining patient and staff privacy pursuant to HIPAA, assuming primary responsibility for caseload including assessing, planning, coordinating, implementing, and evaluating the plan of treatment, making initial evaluations and re-evaluating patient nursing needs, communicating significant findings to Clinical Supervisor and physicians, and revising plans of care in response to identified patient care issues.

Requirements
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Dependable vehicle, valid driver's license, and current auto insurance
- Must be able and willing to travel 50%
- Associates Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
