About this role
National Home Health is hiring for a dedicated – *Intake Coordinator* to join our team in San Jose.
Manages intakes and discharges from Kaiser while working in collaboration with the Clinical team and insurance company to authorize medical necessity for the continuation of treatment at a residential level.

- Manages intakes and discharges from Kaiser while working in collaboration with the Clinical team to authorize medical necessity for the continuation of treatment.
- Screens all inbound phones and transfer calls as appropriate.
- Establishes and maintains efficient methods of ensuring the medical necessity and appropriateness of residential admissions.
- Scheduling patients with nurses and therapists as requested.
- Providing accurate and clear directions to patients prior to their first visit
- Performs concurrent reviews for Residents to ensure that extended stays are medically warranted and documented in the Resident’s medical record.
- Coordinates and organizes activities of the Utilization Review Department using a length of stay as a guideline.
- Calculates the length of stay and continued-stay days for Residents.
- Refers to Physician, Therapist, Nursing Team and Resident if there is a continuation of treatment and stay.
