Primary Responsibilities and

Duties

Conduct comprehensive assessments of residents’ health status, including vital signs, medical history, and current symptoms. Collaborate with residents, families, physicians, and other team members to establish goals of care and develop appropriate care strategies. Regularly reassess resident needs and adjust care plans accordingly, ensuring continuity and responsiveness to changing conditions.

Provide skilled nursing interventions and treatments in accordance with physician orders and established care protocols. Administer medications, perform wound care, monitor vital signs, and manage medical equipment as needed to support resident health and recovery. Coordinate with ancillary services, including therapy, dietary, and social services to ensure comprehensive and integrated care delivery.

Educate residents and families on health conditions, treatment options, medication management, and self-care strategies to promote understanding and empowerment. Provide emotional support, counseling, and advocacy to residents and families facing health challenges, fostering a trust and therapeutic relationship. Promote preventive health practices and wellness initiatives to optimize resident health and quality of life.

Document nursing assessments, interventions, and outcomes in resident medical records. Participate in care conferences and team meetings to discuss residents’ progress goals, and any changes in condition or care needs. Adhere to infection control protocols and maintain a clean, safe, and hygienic environment for residents, staff and visitors.

Collaborate with other healthcare professionals, including registered nurses, physicians, therapists, and social workers, to optimize resident outcomes and promote team work. Perform all other duties assigned.
