Record vital signs, report adverse medication/treatment reactions, and administer injections as prescribed. Assess symptomology and provide crisis intervention and medication education.

Responsibilities
Reasonable accommodation may be made for individuals with disabilities to perform essential job functions. Provide injections, medication support, diagnostic reviews, case consult, crisis intervention, and targeted case management services to assist clients with basic needs and symptom management. Vitals, health and wellness screenings/assessments, wellness coaching and education.
Accurate and timely documentation as contractually required and meet minimum billable requirements for direct client services. Dispense and account for prescribed medication, assist psychiatrist in administering and evaluating prescribed medication and arrange lab work according to protocol. Ensure doctor’s orders are carried out and coordinate medication orders and refills with pharmacies.
Shift coverage including part of the on-call team. Provide field-based services as required by program and client needs. Maintain confidentiality of client care.
Participate in scheduled program meetings. Attend Quarterly Medication Monitoring Committee. Other relevant duties as assigned.
Qualifications
Licensed Vocational Nurse in California is required. Valid CA Driver’s License and current auto insurance per contract requirement required. Experience providing mental health and/or substance use disorder services preferred. Cultural competency understanding is required. Bilingual preferred. Knowledge of Microsoft Office and EHR software preferred.
