About this role

Licensed Practical Nurse/LPN, Full-Time, 1.0 FTE Night Shift, Racine, WI Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. We are offering rewarding opportunities to provide care and service in peaceful, tidy and a well-supplied residential setting that is home to a sister community we serve.

The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a full-time opportunity on our day shift for Racine - Belle City, 5200 Washington Ave., Racine, WI 53406.

Responsibilities

  • Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday operations.
  • Serves as coordinator of care for scheduled weekend and holiday shifts.
  • Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.

Education

Diploma from accredited program in practical nursing required.

Licensure

A current Wisconsin Licensed Practical Nurse/LPN license in good standing

Certification

Current CPR/AED certification

Schedule

  • Hours of Operation, 8:00 am - 4:30 pm (40 hours per week)
  • primary work is in clinic but will need to make home visits also
