Your Role as a Home Health RN
- Perform comprehensive patient assessments in the home setting
- Develop and manage individualized care plans
- Provide skilled nursing care and patient/caregiver education
- Administer treatments and monitor the patient's condition
- Coordinate care with physicians and interdisciplinary teams
- Evaluate patient progress and adjust care as needed
- Complete, accurate, and timely documentation using streamlined tools
