Day to day: provide prescribed treatment and health care procedures as ordered by a physician Provide prescribed treatment and health care procedures as ordered by a physician
Day to day: advise supervisor of all concerns and issues related to nursing services Advises supervisor of all concerns and/or issues related to the provision of nursing services
Day to day: create and distribute Individual Service Plan Medical/Health Reports Create and distribute Individual Service Plan Medical/Health Reports detailing participant’s general health, health care goals, and physician recommendations for each participant
Day to day: document individual health status for support people, family, guardian, and staff Document on the individual’s health status for various support people including family, guardian, staff, employment and other professional staff as necessary
Day to day: record detailed care reports reflecting therapeutic plan in electronic health record Records a detailed report of care given which reflects the therapeutic plan and specific aspects of the patient care plan on individual’s official electronic health record

Pay Rate: $34 to $40/hour
