Responsibilities: Provide skilled nursing care and case management to home health patients; Conduct assessments, manage medications, and update individualized care plans; Collaborate with physicians, therapists, and other care team members; Educate patients and caregivers on disease management and home care techniques; Maintain timely and accurate clinical documentation.
