We’re seeking a dedicated Nurse Navigator - RN - to be part of this exciting movement, helping to bring much-needed support and guidance to underserved populations across the community. Being a Nurse Navigator RN provides a unique opportunity to work onsite in a collaborative environment, coordinating care and guiding patients through their health journeys without direct bedside care. You’ll be at the forefront of a healthcare revolution that uses technology and teamwork to make a real, lasting impact. What You Will Do Be a Catalyst for Change: Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes. Telephonic Patient Assessments: Assess patient needs remotely using evidence-based protocols, helping them navigate the complex healthcare system. Empower & Educate: Provide vital health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum. Leverage Technology: Engage in telemonitoring activities, utilizing cutting-edge tools to track progress and intervene proactively. Lead & Collaborate: Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery. Promote Our Mission: Represent GMR’s innovative programs within the community, reaching populations that need care most but often go underserved. Facilitate & Educate: Serve as a facilitator and educator for GMR’s integrated healthcare training, empowering others to deliver better care. Deliver Excellence: Manage your time, resources, and documentation efficiently while providing outstanding customer service and support. Your work will directly contribute to creating a healthier, more equitable community. Minimum Required Qualifications Valid RN Compact State License. Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment. Experience with clinical data collection, ethical principles, and patient advocacy. Graduate of an accredited nursing program. Current RN license in a compact state. Current CPR certification. Bachelor’s or master’s degree in healthcare administration, Public Health, or related fields is a plus, especially if focused on community health or disease education preferred.
