About this role
Cleveland Clinic is pleased to offer the position of RN Care Coordinator in the Department of Hematology & Medical Oncology. As part of the Cleveland Clinic team, your role will focus on coordinating care for high-risk patients, working collaboratively with a multidisciplinary team to maximize patient outcomes. Responsibilities will include conducting comprehensive clinical assessments, providing care and disease management coordination, and serving as a liaison for patients and families.
Minimum qualifications include a graduate from an accredited school of Professional Nursing and current state licensure as a Registered Nurse. Preferred qualifications include a Bachelor’s of Science in Nursing and certification in relevant specializations. This position involves providing comfort and reassurance to patients during their healthcare journey, optimizing communication and transitions of care, and ensuring timely and efficient care delivery.
