We are seeking a dedicated and compassionate LVN/LPN to join our in-home wound care team. - Conduct comprehensive patient assessments, including vital signs, wound evaluations, and overall health status to inform treatment plans.
- Administer wound care treatments such as dressing changes, ultramist, and infection control measures following best practices.
- Document all patient interactions accurately using Electronic Health Record (EHR) systems (NextGen, google sheets, roam) ensuring compliance with HIPAA and confidentiality policies.
- Educate patients and caregivers on wound management techniques, medication schedules, and infection prevention strategies to promote healing at home.
- Collaborate with interdisciplinary teams including physicians, case managers, and other healthcare professionals to coordinate ongoing care.
- Maintain sterile techniques during procedures such as catheterization or specimen collection and ensure all equipment is properly sterilized and maintained.
*Qualifications*
- Valid LVN/LPN license with current state certification required.
- Will train on wound care and Ultramist use, but experience in basic wound care and dressing changes is preferred.
- Home health or Hospice background highly preferred
- Strong knowledge of anatomy, physiology, infection control protocols, and aseptic techniques.
- Ability to perform physical examinations, and clinical assessments confidently.
- Excellent communication skills for effective caregiver education and interdisciplinary collaboration.
