Responsibilities

  • Align care with Elara Caring mission and leadership direction
  • Maintain HIPAA confidentiality standards
  • Assess patient and family needs (physical, psychosocial, environmental)
  • Manage full caseload including planning, coordinating, and evaluating care plans
  • Perform initial and ongoing patient evaluations and update care plans
  • Communicate changes in patient condition to clinical leadership and care team
  • Revise and coordinate care plans with interdisciplinary team

  • Current, unrestricted RN license
  • 1+ year clinical nursing experience
  • Willingness to travel within assigned territory (up to 50%)
  • Valid driver’s license, reliable vehicle, and current auto insurance
  • Ability to lift/push/pull up to 50 lbs and assist with patient mobility
  • Associate’s or Bachelor’s in Nursing preferred • Home Health, Hospice, or HomeCare HomeBase experience preferred
