You’ll collaborate with physicians, interdisciplinary teams, and community health professionals to ensure seamless, compassionate care. Key Responsibilities: • Assess and evaluate patient status • Develop and revise individualized care plans • Provide palliative nursing care and education • Act as Case Manager when assigned • Maintain accurate clinical documentation • Communicate with physicians and community partners Qualifications Homecare Homebase/Point Care EMR experience is an added bonus. This role is ideal for someone who thrives in a patient-centered environment and values meaningful connections.
