Duties

What you will do

  • Continue to develop Unity Hospice’s palliative care program
  • Manage a caseload of palliative patients
  • Assist with all aspects of patient care including: diagnosis, treatment, medication management, assessments, and consultations
  • Maintain comprehensive knowledge in field of hospice and palliative care
  • Travel to patients wherever they reside: Home, nursing facility etc.

Requirements

What you need to have

  • Valid Advanced Practice Registered Nurse (APRN) license
  • Valid Registered Nurse (RN) license
  • NPI number, or ability to obtain one
  • Controlled Substance License and DEA Number (preferred)
  • Willingness to enter into collaborative agreement with Hospice Medical Director
  • Driver’s license, insurance and vehicle
  • 1-2 years of nursing experience (preferred)
  • Utmost compassion and integrity
