About the Role
As a Wound Care Nurse (RN/LPN), you will be responsible for assessing, treating, and educating patients with acute and chronic wounds. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans that promote optimal wound healing and improve patient outcomes. Your expertise in wound care management will be essential in preventing and managing wound-related complications.

Responsibilities
Assess and evaluate patients with acute and chronic wounds, including wounds caused by diabetes, vascular disease, and pressure ulcers
Develop and implement individualized care plans to promote wound healing and prevent further complications
Provide wound cleansing, debridement, and dressing changes as needed
Conduct regular assessments to monitor wound progress and adjust care plans accordingly
Educate patients and their families on wound care techniques and self-management strategies
Collaborate with other healthcare professionals to ensure comprehensive care and communication
Document patient information and progress in accordance with facility policies and regulations
## Required Skills and
Qualifications
Current certification as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Wisconsin
