*Overview* Heart To Heart Home Care Agency is seeking an experienced, organized, and dependable Registered Nurse (RN) – Clinical Coordinator to join our growing team. This position combines clinical home health visits with clinical administrative responsibilities. The RN will be responsible for completing patient assessments and required clinical visits while also assisting with authorizations, plans of care, physician orders, clinical documentation, and ongoing coordination of skilled home health services. The ideal candidate has experience in home health, skilled nursing, or care coordination and is comfortable working both independently in the field and administratively in the office. Responsibilities * Complete initial nursing assessments and start-of-care visits * Complete recertification, reassessment, and supervisory visits as required * Perform skilled nursing visits when necessary * Develop, review, and update patient Plans of Care * Complete and submit insurance authorizations and reauthorizations * Track authorization expiration dates and ensure timely renewals * Obtain and follow up on physician orders and required signatures * Review clinical documentation for accuracy, completeness, and compliance * Coordinate with physicians, insurance companies, patients, caregivers, and other healthcare professionals * Maintain complete and accurate patient clinical records * Ensure required clinical paperwork is completed within established timeframes * Assist with admissions and discharge documentation * Monitor patient changes in condition and communicate with physicians when necessary * Assist with clinical compliance, quality assurance, and preparation for audits or surveys * Ensure services are provided according to physician orders and the patient’s Plan of Care * Maintain compliance with agency policies and applicable state and federal home health requirements Qualifications * Current and unrestricted Pennsylvania Registered Nurse (RN) license * Home health or skilled nursing experience preferred * Experience completing clinical assessments and nursing documentation * Experience with insurance authorizations and reauthorizations preferred * Strong organizational and time-management skills * Excellent written and verbal communication skills * Ability to manage both clinical and administrative responsibilities * Comfortable communicating with physicians, insurance companies, patients, and families * Strong attention to detail and ability to meet documentation deadlines * Reliable transportation for patient visits Position Job Type: Part-Time with opportunity for growth Schedule: Flexible based on patient and agency needsPay: Starting at $45.00 per hour, based on experience This is an excellent opportunity for an RN who enjoys patient care but also has strong clinical administrative and organizational skills. As our skilled home health program grows, there will be an opportunity for increased hours and additional clinical leadership responsibilities. Pay: $42.20 - $50.83 per hour Work Location: In person
