Do you have a passion for coordinating patient care and improving outcomes? Join the dedicated Case Management team at Grand Strand Medical Center, where you will play a critical role in ensuring patients receive the right care at the right time while helping navigate complex healthcare needs throughout their hospital stay. The RN Case Manager collaborates with physicians, nurses, patients, families, and interdisciplinary teams to coordinate care, facilitate discharge planning, and promote efficient utilization of healthcare resources. This role serves as a patient advocate, helping to ensure quality outcomes, seamless transitions of care, and compliance with regulatory requirements.

Requirements

  • Active and unrestricted Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Current BLS certification
  • Minimum of 2 years of clinical nursing experience in the acute care setting
  • Prior case management experience required
