About this role

The MDS Coordinator is responsible for the timely and accurate completion of the RAI/MDS process from admission through discharge. This role works closely with the interdisciplinary team to ensure regulatory compliance, accurate reimbursement, and high-quality resident care. Key responsibilities include coordinating and managing the RAI/MDS process in compliance with federal and state regulations, collaborating with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews, ensuring accurate MDS coding supported by appropriate clinical documentation, monitoring resident status, maintaining data entry in PointClickCare, submitting MDS documentation to the state, participating in care plan meetings and quality assurance activities, tracking discharges/re-entries/Medicaid/PPS requirements, and reporting audit findings to the Quality Assurance Committee.

Requirements

  • Graduate of an approved LPN or RN Nurse program
  • Active state license required
  • 2+ years of skilled nursing experience preferred
  • Working knowledge of MDS 3.0, RAI process, OBRA regulations, and care planning
  • Knowledge of Medicare PPS, Case-Mix, and Medicaid reimbursement
  • Experience with PointClickCare preferred
  • Strong organizational, documentation, and communication skills
