About this role
Hospice of Florida is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule:
Monday - Friday, 8 am - 5 pm. On-Call as Needed. Territory: Nassau, Duval and Baker. The role involves delivering hands-on hospice nursing care, coordinating services, and guiding patients and families through every step of their hospice journey as a member of an interdisciplinary care team.
Requirements
Current unencumbered license as a Registered Nurse
Minimum one (1) year experience as an RN in a medical surgical/acute care setting
Hospice experience (a plus)
Computer proficient in typing and various programs, including background in EMR
Current CPR certification if required by state
BLS Certification
Current driver's license, vehicle insurance, and access to dependable transportation
Background screening through the Florida Care Provider Background Screening Clearinghouse
