Hospice of Florida is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule
Monday - Friday, 8 am - 5 pm. On-Call as Needed. Territory: Nassau, Duval and Baker. The role involves delivering hands-on hospice nursing care, coordinating services, and guiding patients and families through every step of their hospice journey as a member of an interdisciplinary care team.
