Home Harbor is a 110-apartment residential care apartment complex (RCAC) in Racine, WI. Summary

*

The LPN will provide nursing care, medication management, and health monitoring for residents while supporting the RN and care staff in maintaining high standards of resident care and service.

Reports To

The LPN reports to the Registered Nurse (RN).

Supervision

This role provides support, guidance, and collaboration with caregivers and other care team members.

Responsibilities

• Provide direct nursing care and health monitoring for residents.
• Administer medications and treatments in accordance with physician orders and state regulations.
• Assist with resident assessments and care plan updates under RN supervision.
• Observe and document changes in resident condition, reporting to the RN, physician, and family as appropriate.
• Coordinate care with outside service providers.
• Assess residents for return to the community following hospitalization or rehabilitation center placement.
• Support, guide, and collaborate with caregivers and other team members.
• Maintain accurate medical records and documentation.
• Participate in staff training, quality improvement initiatives, and resident care conferences.

Education

• Current Wisconsin Licensed Practical Nurse (LPN) license in good standing.
• Experience in assisted living, long-term care, or community-based residential care required.
