We are seeking a compassionate and dedicated Palliative Care Registered Nurse to join our dynamic healthcare team.   • Assess patient conditions through comprehensive evaluations, including vital signs, symptom severity, and physiological status.
  • Develop and implement individualized care plans that prioritize pain management, symptom control, and emotional support in accordance with palliative care principles.
  • Collaborate effectively with healthcare professionals across disciplines such as physicians, social workers, chaplains, and therapists to coordinate holistic patient care.
  • Manage medication administration related to symptom relief, ensuring adherence to safety protocols and regulatory standards.
  • Educate patients and families about disease progression, treatment options, and advanced care planning to facilitate informed decision-making.
  • Document all patient interactions accurately within electronic health records (EHR) systems while maintaining compliance with HIPAA regulations.
  • Participate in discharge planning and post-acute care coordination to ensure seamless transitions for patients returning home or moving into hospice or other supportive services.

*Skills*

  • Proven experience in critical care settings such as ICU, emergency medicine, or trauma centers is highly desirable.
  • Strong knowledge of hospital medicine practices including medical-surgical nursing, infectious disease care, and acute care procedures.
  • Familiarity with Level I and Level II trauma center protocols enhances ability to manage complex cases effectively.
  • Experience with palliative and hospice care models alongside skills in managing chronic illness and end-of-life needs.
  • Proficiency in electronic health records (EHR/EHR management), medical terminology, and health regulation policies such as HIPAA.
  • Excellent patient assessment skills coupled with the ability to communicate clearly and compassionately with diverse patient populations including geriatrics and pediatrics.
  • Ability to collaborate seamlessly with multidisciplinary teams while demonstrating strong clinical case management capabilities.
  • Knowledge of discharge planning processes, utilization management, diagnostic evaluation, and post-acute outpatient or home health services.
  • Familiarity with managing patients across various settings—hospital inpatient units, outpatient clinics, or home health environments—ensuring comprehensive patient service delivery.
