Sequoia Hospice Care is seeking a Hospice Nurse/Case Manager to join our core interdisciplinary team. In this role, you will play a key part in assessing patient needs, coordination care, and guiding the delivery of patient-centered services to patients and families. Strong clinical judgement, effective communication, and genuine compassion are essential to ensuring the highest quality of life as we support patients and families through this meaningful stage of life.
Responsibilities include completing comprehensive nursing assessments, developing and managing individualized care plans, coordinating with physicians and the interdisciplinary team, and providing education and support to patients and their families. The RN will also monitor changes in patient condition, manage symptoms, and ensure care is aligned with patient goals and hospice philosophy.
A candidate with experience as a Case Manager in Hospice is preferred.
Pay: $80,000.00 - $90,000.00 per year Work Location: In person
