The Home Health Nurse conducts monthly supervisory visits, assesses client needs, and initiates or updates care plans. Duties include maintaining accurate clinical notes, preparing nursing care plans, and counseling patients and families on nursing needs. The nurse provides skilled, preventive, and rehabilitative care using specialized nursing skills to promote health, independence, and quality of life.

  • Current state license as a Registered Nurse
  • Home Health Care experience desirable
  • Experience with Medicare and/or Medicaid preferred.
  • Ability to use diplomacy
  • Ability to communicate efficiently and effectively both verbally and written
  • CPR required
  • Maintain vehicle insurance
  • Ability to maintain multiple case loads and paperwork
  • Must be able to lift a minimum of 50 pounds
  • Compassion and sincere desire to help patients

Responsibilities

  • Makes the initial evaluation list.
  • Continually re-evaluates the patient’s nursing needs.
  • Initiates plan of care and necessary revisions.
  • Provides those services requiring substantial specialized nursing skills.
  • Provides leadership in team conferences for family teaching and involvement.
  • Initiates appropriate preventive and rehabilitative nursing procedures for the patient and the Agency.
  • Instructs the patient/client and caregiver on all aspects of providing care, disease process and medications.
  • Prepares and keeps current clinical and progress notes.
  • Informs the physician and other personnel of changes in the patient’s condition and needs.
  • Counsels the patient and family in meeting nursing and related needs.
  • Prepares nursing care plan and supervises the care provided.
  • Participates in in-service programs.
  • Supervises and teaches other nursing personnel as appropriate.
  • Participates as a member of appropriate committees as requested.
  • Supervises the care provided by LPN/LVN, Home Health Aides/Homemakers/Personnel care attendants as outlined in CR-10 (a)
  • Prepares written care plan for Home Health Aide/Homemaker/Personnel Care Attendant
  • Participates in on-call rotation
  • All other duties as assigned
  • Uphold company policies and promptly report any misconduct

*Physical & Essential Job Functions:*

  • Ability to communicate effectively through speaking and hearing
  • Must be able to sit for long periods and utilize hands for tasks such as typing, writing, and handling objects
  • Regular attendance and punctuality are required
  • Must be able to stoop, kneel, crouch, stand, and walk as needed
  • Ability to lift, carry, push, or pull objects weighing up to 50 pounds
  • Vision requirements include the ability to see clearly at close range and maintain focus on detailed tasks.
  • Mental capabilities must include the ability to understand and apply both oral and written instructions, demonstrate strong memorization skills, and engage in effective problem-solving.

  • Pay: $28.00 per hour
  • Expected hours: 20 per week
