ABL Healthcare is seeking an experienced, self-driven Part-Time (Per Diem) Registered Nurse (RN) to join their Home Health team in Concord, CA. The RN will evaluate patients' health, create and implement care plans, track patient progress, administer treatments, monitor patient conditions, maintain clinical records, supervise LVNs, and comply with company policies. The position involves working closely with the Clinical Director and Clinical Staff, providing support both remotely and independently in the field.
Requirements
Current Registered Nurse (RN) License to practice in California - Required
Experience in Home Health or Hospice care - 1 Year Required
Current CA Driver's License
Able to travel within a 15-25 mile radius on a regular basis
