About this role
Primary Registered Nurse (RN) - Sturgeon Bay Territory. At Unity Hospice, serve as the lead case manager for patients in both home and facility settings. Partner with float nurses and an interdisciplinary team to deliver consistent, high-quality, patient-centered care.
Full-time, 36 hours per week, consisting of four 9-hour shifts. Weekend rotation every 3rd weekend with 12-hour shifts. Includes shared on-call rotation and 2-3 holidays per year.
Provide compassionate hospice care and case management, assess patient and family needs, respond to urgent situations, collaborate with physicians and interdisciplinary team, educate and support families, administer medications, delegate and guide LPNs and CNAs, and assist with admissions.
