The RN Case Manager is responsible for coordinating and managing the nursing care of individuals with Intellectual and Developmental Disabilities (IDD) in the home setting. This role involves comprehensive case management, including assessing individuals' needs, developing care plans, and ensuring coordination among interdisciplinary teams to ensure quality care for individuals with IDD. The RN Case Manager serves as the primary point of contact for individuals and their families, ensuring that all healthcare needs are met and that services are provided in a timely, efficient, and compassionate manner.

Requirements

  • Bachelor's Degree in Nursing (BSN or ADN) from an accredited nursing program
  • Valid Ohio Registered Nurse (RN) license
  • Minimum of 2 years of nursing experience (home health or IDD experience preferred)
  • Background in case management, care coordination, or interdisciplinary collaboration highly desirable
  • BCI clearance (required) and FBI clearance (if resident of Ohio for less than 5 years)
  • Motor Vehicle Check
  • Authorization to work in the United States
  • Current First Aid and CPR certifications
