Responsibilities • Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews to determine individual care needs. • Administer medications, injections, IV infusions, and perform procedures such as suturing and catheterization with precision and adherence to aseptic techniques. • Collaborate with multidisciplinary teams to develop, implement, and evaluate personalized care plans aligned with patient goals and medical protocols. • Document all patient interactions accurately using the EMR (Electronic Medical Records) system while ensuring compliance with HIPAA regulations. • Provide patient education on health coaching, medication administration, post-discharge instructions, and chronic disease management including geriatrics and dementia care. • Support triage activities in urgent care or emergency room settings by prioritizing patient needs based on severity of condition. Requirements • Valid nursing license (RN) in good standing with relevant state licensing boards. • Proficiency with EMR/EHR systems. • Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding, and laboratory procedures. • Ability to work effectively with individuals with disabilities or developmental disabilities; experience in geriatrics or memory care is a plus. • Excellent communication skills for patient assessment discussions and interdisciplinary collaboration; physical ability to perform duties including lifting Hoyer lifts or assisting with ambulation as needed.
