About this role
The Family Medicine Nurse Practitioner provides advanced nursing care to patients of all ages, from infants to older adults. This role involves assessing, diagnosing, treating, and managing acute and chronic illnesses, promoting health and wellness, and working in collaboration with physicians and other healthcare providers. Conduct comprehensive physical exams and health assessments.
Obtain and interpret patient histories and diagnostic tests. Diagnose and treat common acute and chronic medical conditions. Prescribe medications and therapeutic treatments within the scope of practice.
Develop and implement individualized care plans. Provide health education and counseling on disease prevention and healthy lifestyle choices. Manage follow-up care and coordinate referrals to specialists when needed.
Perform minor procedures as appropriate (e.g., wound care, suturing). Document patient care accurately in the electronic health record system. Participate in quality improvement and practice initiatives.
Collaborate effectively with physicians, nurses, and other healthcare team members. Maintain current knowledge of evidence-based practice guidelines.
