SummitWest Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing services throughout Mesa County. Home health offers a different nursing experience.

Instead of balancing multiple call lights, constant interruptions, and large patient assignments, you'll provide one-on-one care where it matters most: in the patient's home. You'll have the flexibility to build your schedule, the autonomy to work independently, and the support of an experienced clinical team committed to your success.

Responsibilities

  • Provide skilled nursing services in patient homes
  • Perform wound care, medication support, and patient monitoring
  • Observe and report changes or concerns promptly
  • Document visits accurately and timely in the EMR
  • Collaborate with RNs, therapists, caregivers, families, and providers
  • Support patient education and individualized care plan goals

Qualifications

  • Current Colorado Licensed Practical Nurse (LPN) License
  • Valid driver's license, reliable vehicle, and auto insurance
  • IV Certification preferred; not required
  • Strong communication and accurate documentation
  • Ability to work independently and adapt professionally in home health environments
