About this role

Ennoble Care is hiring for a full-time Hospice RN Case Manager to visit patients in the Lancaster / York / Lebanon County, PA region. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
