Responsibilities
Deliver care according to the physician’s plan and established nursing standards. Conduct home visits and shifts, assessing client conditions, recording vitals, and noting any changes. Administer nursing procedures, manage medications, and provide treatments following physician orders and scope of practice. Promote a safe environment by educating families on home safety, infection control, and fall prevention. Maintain clear communication with the RN Case Manager and other team members to coordinate and enhance care quality. Document all care delivery accurately and in a timely manner. Train families in home care practices and collaborate creatively to meet care objectives.

Qualifications
LPN licensed in Iowa. 1-2 years of experience in acute hospital or home health care preferred. Knowledge of nursing standards as set by the Iowa Board of Nursing. Strong communication skills and commitment to confidentiality. Valid driver’s license and reliable transportation.
