About this role
- Complete scheduled home health visits by coordinating working schedule to fit patient's time and needs.
- All visits are to be minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, psycho-social assessment, pain assessment, spiritual assessment, vital signs, administer medications and give treatments as order by physician.
- All patient visit documentation must be completed and submitted electronically no later than 24 hours from time patient visit was completed.
- Attend weekly case conference meetings and any staff meetings upon notification.
- PRN visits to be made as necessary.
- Regularly review and update patient records as they pertain to the care of each patient.
- Complete medication profile updates and case conference documentation no less than weekly.
- All documentation returned for corrections, must be corrected and resubmitted within 24 hours of return.
- Will participate in an on-call rotation to ensure 24 hours nursing care coverage to patients.
Current Licensed Practical nurse license approved by the Oklahoma Nursing Board of Regents.
At least one year experience in either home health setting or acute-care setting with experience in elderly, middle aged, and/or pediatric care.
Able to lift a minimum of 20 lbs without difficulty.
Must own a vehicle in which to make visits and maintain current automobile insurance coverage.
Must have a telephone available for after-hours calls when needed.
