A Licensed Vocational Nurse (LVN) provides essential medical care to patients under the supervision of the RN Case Manager and physicians. Serving patients of all ages—from pediatric to geriatric—LVNs deliver nursing care within their scope of practice, as delegated by the Nurse Case Manager or advanced nursing staff. LVNs may work in various clinical settings, including offices/clinics, onsite recuperative care, and as part of the outreach street medicine team serving unsheltered homeless patients.

Key responsibilities include monitoring vital signs, administering medications, performing routine procedures, maintaining patient records, and communicating with healthcare teams. They also provide emotional support to patients and their families. Working in diverse healthcare environments—such as hospitals, clinics, nursing homes, and private homes—LVNs play a vital role in ensuring quality patient care.

Strong communication skills, empathy, and attention to detail are essential for success in this position.

Essential Duties &

Responsibilities

Provide nursing care for all patients within the scope of LVN practice, including children, adolescents, adults, and geriatrics. Assume responsibility and accountability for assigned patients. Gather data relevant to individual patient needs and age groups.

Document nursing observations, treatments, and patient tolerance per charting standards, ensuring compliance with legal and ethical documentation requirements. Administer medications (oral, injectable, and topical) under supervision. Perform wound care, including dressing changes and cleaning.

Assist with patient mobility and ambulation. Implement basic therapeutic interventions, such as massage, hot/cold therapy, and simple respiratory treatments. Report symptoms, reactions, and patient progress to the Nurse Case Manager or advanced practice nurses.

Follow the proper chain of command to address complex patient care needs. Collaborate with healthcare teams to develop and adhere to patient care plans. Communicate effectively with RNs, physicians, patients, and families regarding care and treatment.

Provide emotional support to patients and families, explaining medical procedures as needed. Utilize educational opportunities to maintain clinical expertise and promote personal development. Stay informed about the legal aspects of nursing practice.

Recognize and respond to patient emergencies, including administering CPR or first aid, and notify RNs or physicians of critical changes in patient conditions. Pursue ongoing education through in-service classes, staff meetings, continuing education, and current professional literature. Perform other duties as assigned.

Education &

Experience

High school diploma or equivalent required. Current and unrestricted LVN in the state of California is required. Must complete 30 hours of continuing education units every two years to renew license.

1-2 years of experience as an LVN in an acute care hospital or community clinic setting is preferred. Experience working with underserved populations facing socioeconomic barriers to healthcare is preferred. Fluency in Spanish is preferred.

Valid, unrestricted CA driver’s license. Certification in Basic Life Support (BLS) from the American Heart Association (AHA) or the American Red Cross preferred. Knowledge and proficiency in Google Workspace, including Gmail, Google Docs, Google Sheets, and Google Calendar, is preferred.

Must be comfortable working in a recuperative care environment.

Must successfully pass a background check.
