Position Overview
The Care Coordination Supervisor provides leadership, clinical coordination, and operational support for an integrated healthcare program. Working closely with program leadership, this role oversees care coordination activities, supports interdisciplinary care teams, supervises staff, and strengthens partnerships across healthcare and community systems.
The ideal candidate is a collaborative leader with strong clinical knowledge, experience managing complex client needs, and a passion for improving health outcomes.
What You'll Do
Leadership & Program Support
Partner with leadership to implement and advance integrated healthcare initiatives. Co-lead multidisciplinary teams including behavioral health providers, nurses, medical assistants, case managers, and other healthcare professionals. Support program operations, workflow development, and quality improvement efforts. Help drive a culture of accountability, collaboration, and excellence.
Care Coordination
Provide care coordination services to individuals with complex medical and behavioral health needs. Connect clients to primary care, specialty care, behavioral health services, and community resources. Monitor chronic health conditions and support treatment adherence and follow-up care. Conduct health screenings and assessments. Track and document key health indicators, laboratory results, and outcomes within the EMR.
Team Supervision & Development
Supervise, mentor, and support care coordination staff, case managers, and medical assistants. Delegate assignments and monitor performance to ensure quality service delivery. Foster professional growth through coaching, training, and ongoing development.
Community & Provider Collaboration
Build strong partnerships with healthcare providers, hospitals, and community organizations. Coordinate information sharing and care transitions to ensure continuity of care. Collaborate with external stakeholders and community partners to support program goals and client success.
Quality & Outcomes
Promote evidence-based practices and whole-person care models. Utilize data and performance metrics to improve outcomes and service delivery. Support program compliance, reporting requirements, and continuous quality improvement initiatives.
Qualifications
Required
RN Candidates
Bachelor's Degree in Nursing (BSN) Active Registered Nurse (RN) license 1-2 years of experience in behavioral health, integrated care, care coordination, or a related healthcare setting
