Position
Summary
You will be responsible for providing nursing care to assigned patients under the direct supervision of a registered nurse (RN). General responsibilities may include giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documentation on patient progress.
Responsibilities
Provide direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of care Observe and report on the home environment and make recommendation that will enhance patient care, promote safety and prevent adverse effects Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomes Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriately Inform Health Care Providers of changes in patient’s condition in accordance with providers orders and documents communications accordingly Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions Provide instructions to patient/CG’s and staff in a timely manner

Qualifications
Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse, and CPR certified At least one year clinical practical nursing experience. *Home Health experience preferred Must have a vehicle, valid current driver’s license, and automobile insurance Ability to make home visits and provide skilled nursing assessments and care in the home
