RN Admissions Nurse – Home Health. Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro. Perform detailed nursing assessments and complete OASIS documentation. Develop individualized plans of care based on each patient's needs and goals. Educate patients and caregivers on medications, diagnoses, and home health services. Coordinate with physicians, therapists, case managers, and the interdisciplinary care team. Help ensure safe, successful transitions from hospital to home.
