Home Infusion Registered Nurse - Part-time West Valley

Provides skilled professional nursing services to a case load of patients in their place of residence in accordance with the patient's plan of care. Assesses patients prior to provision of care. Provides ongoing assessment of the patient and patient's equipment needs.

Initiates timely discharges. Educates and instructs the patient, patient's family, or staff as required. Prepares timely, legible, and complete documentation of all care, interventions, and care coordination.

Demonstrates awareness of and sensitivity to patient/family rights. Applies safety principles. Participates in orientation and training of new staff, care coordination meetings, and continuing education.

Ensures compliance with legal issues including patient confidentiality and risk management. Ensures compliance with JCAHO, federal, state, and local regulations.
