• What You'll Do
• Manage and coordinate patient care plans
• Perform skilled nursing visits and assessments
• Collaborate with physicians, therapists, and the interdisciplinary team
• Educate patients and caregivers
• Ensure quality care and regulatory compliance
• Advocate for patients and families throughout their healthcare journey
• Qualifications

✅ Current Oregon RN license

✅ Strong assessment and communication skills

✅ Home Health or Hospice experience preferred

✅ Current driver's license and reliable transportation
