MDS Coordinator (Part-Time / PRN) at San Francisco Post Acute. Pay Range: $50.00 - $55.00 per hour. Seeking an experienced MDS Coordinator to oversee the resident assessment process and ensure compliance with federal, state, and facility guidelines. Responsible for coordinating MDS assessments, supporting PDPM optimization, monitoring quality outcomes, and collaborating with the interdisciplinary team to deliver exceptional resident care.

Requirements

Active RN license in good standing in the State of California
Graduate of an accredited nursing program; Associate Degree in Nursing or higher preferred
Experience with MDS, PDPM, Medicare reimbursement, and long-term care regulations preferred
Long-Term Care and skilled nursing experience strongly preferred
