Summary
We are offering a $1500 sign-on bonus for this full-time position, and we are eager to welcome a compassionate and skilled LPN to our team. In this role, you will provide direct patient care, develop individualized care plans, monitor and report patient progress, and maintain accurate records. You will also participate in quality improvement initiatives and collaborate with other healthcare professionals to ensure seamless communication and coordination of care.
Responsibilities
- Provide direct patient care, including vital sign checks, wound care, and medication administration, under the supervision of a registered nurse or physician
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report patient progress, identifying areas for improvement and making necessary adjustments to care plans
- Maintain accurate and detailed records of patient care, treatment, and progress
- Participate in quality improvement initiatives to enhance patient care and satisfaction
- Collaborate with other healthcare professionals to ensure seamless communication and coordination of care
