About this role
URGENT HIRING: Nurse Clinical Manager – Broward County
Activa Home Health (an Accord Care Company) is a licensed Medicare-certified home healthcare agency seeking and enthusiastic customer service driven RN Clinical Manager for our Broward office. This position will primarily work in office and will also perform field case management with responsibilities for assessment, coordination of care with other disciplines and communication with referral sources. Coordinate patient admissions with other members of our care team. Must have strong case management experience and working knowledge of Medicare.

- Perform case management oversight, and coordination of care for all skilled disciplines.
- Close communication with social worker and veterans division.
- Coordinate patient admission with other members of the healthcare team. Provide nursing care and treatment of home care patients in accordance with the plan of treatments and approved authorizations. RN will supervise all non-skilled caregivers.
Qualifications
- Current RN Florida professional license.
- Minimum of one year experience with Medicare home health case management.
- Must have excellent time management skills to prioritize and organize workload.
- Positive attitude and strong work ethic.
- Excellent communication and customer service skills.
- Experience with Kinser Wellsky a plus.
