About this role
HealthFlex is seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager who strives to make a real impact for our community. By joining our team you will be providing skilled services to help patients and families cope with the End of Life process. Responsibilities include completing initial and ongoing assessments, providing professional nursing care, writing and initiating plans of care, administering medications and treatments, acting as Case Manager, and working with interdisciplinary group members.
