About this role

The Community Health Nurse Clinician is a member of the Community Programs team and provides nursing care and care coordination for individuals residing in a shelter setting. Working as part of an interdisciplinary team, this role supports patients experiencing homelessness, housing instability, and other social barriers to health through relationship-based, patient-centered care. Provides assessment, chronic disease management, patient education, clinical interventions, and care coordination within the shelter environment and across community healthcare resources as needed.

Utilizes a trauma-informed, harm-reduction approach to promote health, increase access to care, and support patients in achieving their healthcare goals.

Requirements

  • AAST or BSN degree in Nursing from an accredited college or university
  • One (1) year of experience required
  • Unencumbered Washington State Registered Nurse license required
  • National Certification in Ambulatory Care or similar type certification desired
  • Current immunizations
  • Experience with EPIC EHR
  • For Nurse Clinician III: BSN and minimum of 5 years professional nursing experience or equivalent totaling 15 years
