MDS & Assessment Coordination

Role

Overview

Complete and coordinate all MDS assessments (admission, quarterly, annual, significant change).

Ensure accuracy and timely submission of MDS assessments in compliance with CMS guidelines.

Participate in interdisciplinary team meetings related to resident assessments.

Care Planning

Collaborate with nursing, therapy, and other departments to develop individualized care plans.

Ensure care plans reflect residents’ current needs, goals, and preferences.

Update care plans based on assessment findings and changes in condition.

Regulatory Compliance

Maintain compliance with all federal, state, and facility regulations related to MDS and care planning.

Prepare for surveys and audits by ensuring documentation is complete and accurate.

Stay updated on CMS/RUG/PDPM guidelines and changes.

Clinical Documentation

Review medical records to ensure supporting documentation aligns with MDS coding.

Educate nursing staff on proper documentation practices.

Identify and correct discrepancies in clinical documentation.

Reimbursement & Quality Measures

Monitor case mix index (CMI) and reimbursement trends.

Assist in maximizing reimbursement through accurate coding.

Track and report quality measures and outcomes.

Collaboration & Communication

Serve as a liaison between nursing, therapy, and billing departments.

Communicate effectively with physicians and other healthcare professionals.

Participate in Quality Assurance and Performance Improvement (QAPI) activities.
