RN Case Manager – Hospice (Part-Time). Reports To: Manager, Clinical Operations. Location: DFW, TX (Fort Worth, Arlington, Mansfield, and South Dallas). At Hospice Care Partners, managing a caseload of approximately 8–10 patients per week, providing expert nursing care for home and facility-based hospice patients, serving as the primary clinical and communication lead, coordinating care with families, physicians, facility staff, and the interdisciplinary team.
