Team Select Home Care is seeking an organized, relationship-driven Case Manager to join our team. This position serves as a key point of contact for patients and families while coordinating with caregivers and internal teams to ensure consistent, high-quality care and a positive patient experience.
Responsibilities
Serve as the primary point of contact for assigned patients and families.
Build and maintain strong relationships with patients, families, caregivers, and members of the care team.
Coordinate patient services, caregiver schedules, and ongoing staffing needs.
Maintain consistent communication with patients and families regarding scheduling, staffing, and changes in care.
Conduct regular follow-up with patients and families to ensure satisfaction and identify changing needs.
Partner with recruiting and clinical teams to fill open shifts and maintain continuity of care.
Assist with new patient admissions, onboarding, and coordination of services.
Proactively identify and resolve patient, family, caregiver, or scheduling concerns.
Communicate patient updates and changes to the appropriate clinical and operational team members.
Maintain accurate and timely patient documentation and records.
Support caregiver retention by maintaining positive relationships and addressing concerns as needed.
Ensure services are provided in accordance with company policies, procedures, and applicable regulations.
Provide exceptional customer service to patients, families, and caregivers.
Qualifications
Must have an active Registered Nurse (RN) license in good standing in the state of Colorado
Strong communication, interpersonal, and relationship-building skills
Excellent organization, time management, and attention to detail
Ability to prioritize and manage multiple responsibilities in a fast-paced environment
Strong problem-solving skills with a proactive and solutions-focused approach
Ability to collaborate effectively with clinical, recruiting, and operational teams
Strong follow-through, professionalism, and accountability
Previous experience in home care, case management, private duty nursing, or a related healthcare setting preferred
