Team Select Home Care

Team Select Home Care is seeking an organized, relationship-driven Case Manager to join our team. This position serves as a key point of contact for patients and families while coordinating with caregivers and internal teams to ensure consistent, high-quality care and a positive patient experience.

  • Serve as the primary point of contact for assigned patients and families.
  • Build and maintain strong relationships with patients, families, caregivers, and members of the care team.
  • Coordinate patient services, caregiver schedules, and ongoing staffing needs.
  • Maintain consistent communication with patients and families regarding scheduling, staffing, and changes in care.
  • Conduct regular follow-up with patients and families to ensure satisfaction and identify changing needs.
  • Partner with recruiting and clinical teams to fill open shifts and maintain continuity of care.
  • Assist with new patient admissions, onboarding, and coordination of services.
  • Proactively identify and resolve patient, family, caregiver, or scheduling concerns.
  • Communicate patient updates and changes to the appropriate clinical and operational team members.
  • Maintain accurate and timely patient documentation and records.
  • Support caregiver retention by maintaining positive relationships and addressing concerns as needed.
  • Ensure services are provided in accordance with company policies, procedures, and applicable regulations.
  • Provide exceptional customer service to patients, families, and caregivers.

Qualifications

  • Must have an active Registered Nurse (RN) license in good standing in the state of Colorado
  • Strong communication, interpersonal, and relationship-building skills
  • Excellent organization, time management, and attention to detail
  • Ability to prioritize and manage multiple responsibilities in a fast-paced environment
  • Strong problem-solving skills with a proactive and solutions-focused approach
  • Ability to collaborate effectively with clinical, recruiting, and operational teams
  • Strong follow-through, professionalism, and accountability
  • Previous experience in home care, case management, private duty nursing, or a related healthcare setting preferred

Job Type: Full-time Pay: $70,000.00 - $85,000.00 per year
