*Job Summary* We are seeking a dedicated and energetic PACE Intake Nurse to join our dynamic healthcare team. In this vital role, you will be responsible for conducting comprehensive patient assessments, coordinating intake processes, and ensuring seamless transitions into our PACE (Program of All-Inclusive Care for the Elderly) services. Your expertise in acute and post-acute care settings will help us deliver exceptional, person-centered care to our diverse patient population. This position offers an exciting opportunity to make a meaningful impact on the lives of seniors by facilitating access to comprehensive healthcare services through efficient intake and case management practices.

*Duties*

  • Perform detailed patient assessments, including vital signs, medical history, and current health status, to determine eligibility for PACE services
  • Coordinate intake procedures by collecting and reviewing medical records, diagnostic evaluations, and discharge summaries from hospitals, clinics, and other healthcare facilities
  • Collaborate with interdisciplinary teams to develop individualized care plans tailored to each patient’s needs, including acute care, outpatient services, and post-acute care planning
  • Conduct utilization management by evaluating hospital admissions, emergency room visits, and outpatient appointments to optimize resource use and ensure appropriate care levels
  • Facilitate discharge planning by coordinating with hospital staff, primary care providers, specialists, and community resources to ensure smooth transitions across care settings
  • Maintain accurate documentation of patient interactions in medical records while adhering to HIPAA regulations and confidentiality standards
  • Support case management activities by monitoring patient progress, adjusting care plans as needed, and advocating for patient service needs across various healthcare environments such as inpatient units, clinics, or home-based settings

*Experience*

  • Proven experience in critical care environments such as ICU (Intensive Care Unit), emergency medicine departments, or Level I/II trauma centers
  • Strong background in hospital medicine with knowledge of medical terminology, physiology, and diagnostic evaluation techniques
  • Prior experience with inpatient and outpatient settings including post-acute care facilities or clinics serving geriatric or pediatric populations
  • Familiarity with managed care processes, utilization management protocols, and case management strategies within hospital or community health systems
  • Hands-on experience in patient assessment procedures involving vital signs measurement and comprehensive health evaluations
  • Knowledge of behavioral health considerations and infectious disease care relevant to elderly or complex patients
  • Ability to navigate electronic medical records efficiently while maintaining compliance with HIPAA standards
