We are seeking an experienced Registered Nurse Clinical Supervisor to join our leadership team. The ideal candidate is an organized clinical leader with extensive home health experience, strong knowledge of OASIS, and a passion for supervising and mentoring clinicians and ensuring outstanding patient outcomes.

Responsibilities
- Supervise and support a team of Registered Nurses, Licensed Practical Nurses, and other clinical staff.
- Review and approve OASIS assessments and clinical documentation.
- Ensure compliance with Medicare Conditions of Participation, Illinois Department of Public Health regulations, and agency policies.
- Conduct chart audits and implement quality improvement initiatives.
- Assist with patient care coordination and clinical case conferences.
- Mentor, coach, and evaluate field clinicians.
- Participate in orientation and competency evaluations for new clinical staff.
- Collaborate with physicians and interdisciplinary team members to ensure quality patient care.
- Assist with survey readiness, accreditation activities, and performance improvement programs.
- Provide clinical guidance regarding complex patient cases.
- Participate in on-call clinical leadership rotation as needed.
Qualifications
Required
- Current Illinois Registered Nurse (RN) license.
- Minimum of three (3) years of home health experience.
- Minimum of one (1) year of supervisory or leadership experience.
- Strong knowledge of OASIS, Medicare regulations, and home health documentation.
- Excellent communication, leadership, and organizational skills.
- Valid driver's license and reliable transportation.
Preferred
- BSN preferred.
- Homecare Homebase (HCHB) experience.
- QA/Performance Improvement experience.
- ICD-10 coding knowledge.
- Previous survey preparation experience.
Job Type: Full-time Pay: $75,000.00 - $90,000.00 per year
