RN Visit Nurse – Home Health | St. Paul Territory | $4,000 Sign-On Bonus. Full-Time with On-Call and Weekend Rotation.
Compensation:
$75,000 to $100,000 per year plus a $4,000 Sign-On Bonus. Provide skilled RN home health visits throughout the St. Paul territory. Perform comprehensive nursing assessments. Deliver wound care, medication management, injections, and disease-specific treatments. Educate patients and caregivers on medications, diagnoses, and recovery plans. Coordinate care with physicians, therapists, and interdisciplinary team members. Monitor patient progress and communicate changes in condition. Complete accurate documentation using Homecare Homebase. Help patients remain safely at home while reducing avoidable hospitalizations.

Requirements
- Active Minnesota Registered Nurse (RN) license
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
- Strong clinical judgment and assessment skills
- Excellent communication and patient education abilities
- Self-motivated and comfortable working independently
- Reliable transportation and willingness to travel throughout the St. Paul service area
- Rotating weekends and on-call availability
- Mid-level experience
